How to Sign Up and Complete the Registration Process on AppZHealth

This guide will walk you through the sign-up and registration process on AppZHealth.

Step 1: Accessing the Sign-Up Page

1. Open your preferred web browser (Chrome, Firefox, Edge, etc.).
2. Navigate to the AppZHealth website: https://dev.appzhealth.com
3. On the Login screen, click the "Sign Up" button located below the "Continue"
   button.

Step 2: Filling Out the Sign-Up Fields

On the Sign-Up page, enter the following details:

• Email Address: Enter your valid email.
• Password
  : Create a secure password (minimum 8 characters, must include a number or symbol).

✅ Alternative Sign-Up Options: You can also sign up using Google, Facebook, or LinkedIn.

Step 3: Submitting Your Sign-Up Details

1. Review the details you entered for accuracy.
2. Click the "Continue" button.
3. You will be redirected to the Registrationscreen.

Step 4: Completing the Registration Form

On the Registration Page, fill in the required and optional fields:

Required Fields (marked with *):

• First Name*: Enter your first name.
• Last Name*: Enter your last name.
• Email Address*: Use the same email as in Step 2 (for verification).
• Phone Number*: Enter your phone number.
• Doctor NPI*: Enter your National Provider Identifier (NPI).

Optional Fields:

• Practice NPI: Enter your Practice NPI.
• Practice Address: Provide your practice location.
• Practice City & State: Specify your practice's city and state.
• Job Title
  : Enter your job role.

Step 5: Submitting Your Registration

1. Double-check all the information you provided.
2. Click the "Register Now"
   button to complete the registration process.

✅ Once registered, you will receive a confirmation email. Follow the instructions to activate your account.
